Apply for a Seeing Eye® dog

How to Apply

Prospective new students may apply either by completing an online application or by calling to request a paper application at (800) 539-4425.

As a general guideline, the process includes the following steps:

  • Submit an application (online or paper).
  • After submitting an online application, you will receive an automated acknowledgment email with attachments and further instructions. If you do not receive this email, please check your spam folder.
  • Once your online application is submitted, all listed personal references with email addresses will be contacted automatically. Orientation & Mobility instructors will also be contacted via email, so please be sure to include their information.
  • Application materials are collected and reviewed.
  • If all your submitted information looks promising, an instructor will schedule a personal visit.
  • Your completed file is reviewed by the Admissions Committee.
  • You will be notified of the decision.
  • Class dates are finalized based on space as well as availability of best-suited dog.

The time involved in the process varies, depending on these factors:

  • How quickly application materials are received, including all forms and letters of reference. 
  • Availability of an instructor for your personal visit.
Generally, once an applicant is accepted, class placement is within six months to a year.
